& Self-Directed Program Waiver
By purchasing this program, I confirm the following:
Voluntary, Self-Directed Participation. I am voluntarily choosing to participate in a self-directed digital training program offered by Integrated Performance and Health (“IPH”), to be performed on my own, without a coach present to supervise, correct my form, or monitor my condition in real time. I confirm I am 18 years of age or older.
Assumption of Risk. I understand that physical exercise carries real risk, including but not limited to muscle strains, sprains, joint injuries, falls, overexertion, and — in rare cases — serious cardiovascular events. I understand these risks may be greater in a self-directed setting where no coach is present to observe my form or condition, and I voluntarily accept that risk.
Release of Liability. I release, waive, and discharge IPH, its owner, coaches, and employees from any and all liability, claims, or causes of action arising from my participation in this self-directed program, to the fullest extent permitted by Florida law.
Medical Disclaimer. IPH coaches are not physicians, physical therapists, or medical providers. Nothing in this program is medical advice, diagnosis, or treatment. I confirm I know of no medical reason I should not participate, or that I have obtained clearance from a physician to do so. I understand I should stop immediately and consult a physician if I experience pain, dizziness, chest discomfort, or any concerning symptom.
Governing Law. This waiver is governed by the laws of the State of Florida, consistent with IPH’s Terms & Conditions.
